Becoming a VR Vendor

Any items that a client wishes to ask VR to pay for must come from a vendor that has been registered with VR.  VR cannot make purchases from vendors that are not registered and in their system.  This is a very simple process and does not cost anything.
Registering as a VR Vendor boils down to three steps and will take about 15 minutes to complete.

  1. Register online with MyFloridaMarketPlace
  2. Online completion of a Substitute W-9 Form with the State of Florida
  3. Complete a paper registration for VR


All information and forms can be accessed from the VR website.  Follow this link for complete instructions.  About half way down on the page you will see the VR Standard Vendor Application (RTF) link.  This is the paper registration you will need to fill out (#3 above). It is very important that you follow the directions or your registration will not be accepted. 


Fax, mail or scan and email your completed registration form to the VR Vendor Registration office.  Contact information is listed on the application.  Please copy Karen or let her know that your application has been submitted so that she can follow the status of your registration.
